Liberty Village of Centralia is looking for a high-energy and compassionate individual to lead and coordinate daily activities for Alzheimer’s & advanced stage dementia residents as a Memory Care Certified Nursing Assistant (CNA). We have openings for 1st shift. The Memory Care Certified Nursing Assistant (CNA) works closely with the Memory Care Coordinator to develop a plan of activities both in and out of the facility, while ensuring both group and individual engagement through weekly lesson plan and calendar. Individuals must be able to lead exercises, group activities, specialized individual activities, drive facility transport for fields-trips. Creativity is important.
Memory Care Certified Nursing Assistant (CNA) Primary Responsibilities:
1. Provide residents with the required services as listed on the day-to-day CNA duties.
2. Complete all CNA documentation as assigned and ensure that said documentation accurately reflects services as provided to the resident.
3. Provide continuous restorative nursing in conjunction with the resident’s activities of daily living.
4. Implement and deliver scheduled Activity and Ability based programming.
5. Attend various in-service training meetings to maintain skills. Training must include a minimum of 12 hours per year. To meet Memory Lane requirements, training must include the 4 hour specialized Alzheimer’s Orientation Course as well as the 12 hour additional training related to Alzheimer’s and other dementias. A minimum of 12 additional hours of ongoing training must be attained annually.
6. Participate in skills performance reviews as directed by the facility, to be completed at least annually.
7. Perform other duties as assigned as assigned by the Director of Memory Care.
