A full-time Assessor position is now available with Liberty Healthcare. This role involves administering structured assessments to help determine individuals’ eligibility for home and community-based services that support independence and wellbeing.
This is a field-based role which will allow you to primarily work remotely from home and require you to travel to client residences throughout the region.
Your daily work will involve
- Leading in-person and virtual meetings with people who have intellectual and developmental disabilities and autism
- Gathering information from individuals and their families and caregivers
- Using standardized assessment tools to help determine individual needs
- Completing structured reports within established deadlines
There is some flexibility in the work schedule, which is primarily Monday through Friday during daytime hours.
