We are currently seeking a compassionate, dependable, and skilled In-Home Personal Patient Care Assistant (CNA) to join our dedicated team.
The In-Home Personal Patient Care Assistant will provide direct support and personal care services to individuals with developmental disabilities within the home and community setting. This role requires experience providing hands-on care, strong communication skills, and the ability to work collaboratively with healthcare professionals, families, and support teams to ensure high-quality, person-centered care.
Essential Duties & ResponsibilitiesDirect Patient Care- Assist clients with bathing, grooming, dressing, toileting, and other personal hygiene tasks within the home and community setting
- Support clients with feeding, meal preparation, and hydration monitoring as needed
- Assist with mobility, transfers, positioning, ambulation, and wheelchair support
- Provide companionship and emotional support while promoting independence, dignity, and safety
- Assist clients with daily living activities and community participation
- Observe and report any changes in clients’ physical, emotional, or behavioral condition to the supervisor or healthcare team
- Maintain accurate and timely documentation of services provided and daily activities
- Ensure confidentiality and professionalism in all client interactions
- Collaborate with physicians, nurses, therapists, case managers, and other healthcare professionals to support individualized care plans
- Follow client-specific care plans, behavioral support plans, and safety protocols
- Participate in team meetings, trainings, and ongoing professional development as required
- Ensure all healthcare practices comply with local, state, and federal regulations
- Maintain a clean, safe, and organized environment for clients
- Adhere to infection control procedures, safety standards, and company policies
- Respond appropriately during emergencies and follow established safety procedures.
- Completion of an accredited Certified Nursing Assistant (CNA) training program required
- Minimum of 1–3 years of CNA experience preferred, especially working with individuals with developmental disabilities, autism, or behavioral health needs
- Current CPR and First Aid certification required
- Strong understanding of basic patient care, including personal hygiene, feeding assistance, mobility support, and activities of daily living
- Knowledge of patient safety practices, infection control, and maintaining client comfort and dignity
- Familiarity with common medical terminology and healthcare documentation practices
- Excellent interpersonal, communication, and organizational skills
- Ability to work effectively both independently and within a multidisciplinary team environment

- Must possess a valid driver’s license, reliable transportation, and current auto insurance
- Must be willing and able to travel within assigned service areas to provide in-home and community-based services
- Ability to pass background check, drug screening, and all required healthcare clearances prior to employment
