Over the past decade, Libera has expanded significantly, including the opening of a new center in Morgantown that provides therapy, housing, and comprehensive wraparound services for young women. As we deepen our level of care and expand our impact, we are seeking an experienced Senior Case Manager to provide intensive case management and care coordination for participants in our SOAR Program.

Position Overview
The Senior Case Manager provides high-quality, relationship-centered case management services to high-risk young women who have experienced trauma, housing instability, system involvement (juvenile justice, child welfare), behavioral health needs, and other significant barriers to stability.
This role requires strong systems knowledge, the ability to manage complex cases, and confidence coordinating care across multiple providers and agencies. The ideal candidate brings significant experience working with high-need populations and is skilled at building trust with vulnerable youth.

SOAR (Supporting Opportunity & Accelerating Resilience) is Libera’s intensive accelerator program designed to help young women stabilize their lives, strengthen mental health, and build pathways to education and employment. This position ensures participants receive consistent, coordinated support to overcome barriers and move toward independence, resilience, and long-term well-being.
General Duties & Responsibilities
- Conduct comprehensive needs, risk, and strengths assessments
- Develop and monitor individualized success and accountability plans
- Provide ongoing, relationship-centered case management using trauma-informed and strengths-based approaches
- Coordinate services across systems, including:
- Behavioral health providers
- Housing and supportive living services
- Workforce and education programs
- Healthcare providers
- Child welfare, probation, or other system partners (when applicable)
- Support participants in accessing housing, employment, transportation, education, and essential resources
- Provide crisis response, de-escalation, and safety planning as needed
- Maintain timely, accurate, and confidential documentation
- Recruit, screen, and onboard SOAR participants
- Facilitate or coordinate workshops focused on life skills, career readiness, and wellness
- Support and monitor mentor-participant relationships
- Track participant progress and program outcomes; prepare monthly, quarterly, and annual reports
- Collaborate closely with Libera therapists and program staff to support whole-person care
- Build and maintain strong relationships with referral sources, community partners, employers, and training providers
- Support research and evaluation efforts related to program outcomes
- May provide supervision for MSW interns or support onboarding and training of new staff as the organization grows

Qualifications & Skills
- Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, or a related field
- Active Social Work license in West Virginia (LGSW or higher preferred)
- Minimum 5 years of experience providing case management or care coordination for high-risk youth or young adults
- Experience managing complex cases involving multiple systems and high-acuity needs
- Strong assessment, documentation, and care coordination skills
- Knowledge of community resources and service systems in West Virginia
- Strong organizational skills and ability to manage multiple priorities
- Excellent relationship-building and communication skills
Preferred Qualifications
- Experience working with housing instability, justice-involved youth, or young adults transitioning to independence
- Experience in community-based or field-based case management
- Experience supervising interns or junior staff
- Strong understanding of trauma-informed care in rural or resource-limited settings
Additional Requirements
- Reliable transportation and ability to travel locally
- Travel across West Virginia for meetings or partner visits (1–4 times per month)
- Occasional evening or weekend hours based on participant or program needs
- Successful completion of background check, Libera Listener Training, and suicide prevention training
Pay & Benefits
- Salary: $60,000–$65,000 per year
