Position Summary
Seeking a compassionate Case Manager to support individuals experiencing homelessness in achieving housing stability and self-sufficiency. Ideal candidates are committed to meeting people where they are, utilizing trauma-informed care and harm reduction.
Responsibilities
- Conduct client intakes, assessments, and service planning.
- Connect clients to housing, healthcare, behavioral health, employment, and community resources.
- Provide ongoing case management, crisis intervention, advocacy, and housing support.
- Maintain accurate case notes, client records, and program documentation.
- Utilize motivational interviewing, trauma-informed care, and harm reduction practices.
- Collaborate with community partners and multidisciplinary teams.
Qualifications
- A minimum of 1 year of experience in case management, homeless services, healthcare, behavioral health, social services, or related field preferred.
- Passion for serving vulnerable populations and supporting housing stability.
- Valid driver's license, reliable transportation, and auto insurance.
- Ability to pass background check, drug screen, and TB test.
- CPR/First Aid certification or willingness to obtain.
Pay: $26–$31/hr
Schedule: Sun–Thurs, 9:00 AM–5:30 PM
Location: San Jose, CA 95121 Onsite
Type: Full-Time | Open-Ended Contract
