The Case Manager plays a crucial role in the Care Services industry by coordinating comprehensive care for clients facing various challenges. Minimum Qualifications:

  • Bachelor's degree in Social Work, Psychology, or a related field.
  • Must be fluent in Spanish
  • Experience in case management or social services, particularly in a health care setting.
  • Strong understanding of mental health issues and crisis intervention techniques.

Preferred Qualifications:

  • Master's degree in Social Work or a related field.
  • Certification in case management or a related specialty.
  • Experience working with child/family welfare programs or in program development.

Responsibilities:

  • Conduct thorough assessments of clients' needs and circumstances to develop personalized care plans.
  • Coordinate and monitor services provided to clients, ensuring they receive appropriate support and resources.
  • Facilitate communication between clients, families, and service providers to promote collaboration and understanding.
  • Implement crisis intervention strategies as needed to address urgent client situations effectively.
  • Evaluate and adjust care plans regularly based on client progress and changing needs.
