Title: Director of Care Coordination & Social Services

Effective Date: March 2, 2026

Supervisor: Chief Operating Officer/Chief Nursing Officer

Department: Nursing Service

FLSA Status: Exempt


Principal Duties and Responsibilities


  1. Provides leadership and oversight of transitional care, discharge planning, utilization review, swing bed coordination, chronic care management, and social services.
  2. Ensures patient-centered, interdisciplinary coordination of care across the continuum, addressing clinical, psychosocial, financial, and community resource needs while monitoring quality, safety, and regulatory compliance.
  3. Coordinates care and discharge planning of hospitalized patients through multidisciplinary rounds, huddles, and discharge planning meetings.
  4. Assesses and addresses patient discharge needs including clinical, financial, emotional, spiritual, and community resource needs.
  5. Manages and oversees swing bed services, utilization review, and chronic care management programs.
  6. Provides or coordinates patient and family education related to discharge planning and post-hospital care.
  7. Oversees quality, utilization, and performance data including readmissions, core measures, Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS), and population health metrics.
  8. Leads hospital committees including discharge planning, hospital acquired conditions and utilization review.
  9. Collaborates with nursing leadership, medical staff, and executive leadership to develop and evaluate programs and services.
  10. Develops and manages departmental budgets including staffing, operating costs, equipment, and supplies.
  11. Ensures compliance with regulatory standards, infection control practices, and patient safety initiatives.
  12. Provides direct supervision of care coordination, utilization review, social services, and related staff.
  13. Serves as liaison between the hospital and community agencies and resources.
  14. Participates in patient care activities that could include blood exposure and risk to bloodborne pathogens (eg: lacerations, -handling of blood-contaminated specimens, etc.).
  15. Responsible for ensuring the environment meets appropriate governing body standards.
  16. Ensure patients and visitors follow current infection control guidelines.\
  17. Maintains patient and staff safety through the use of patient safety tools (TeamSTEPPS, Just Culture, etc.).
  18. Serves on various committees as assigned.
  19. Regular attendance at the assigned work location is required.
  20. Performs all other duties as assigned.

Minimum Knowledge, Skills, and Abilities


  1. Knowledge and understanding of nursing theory and practice and the growth and development of a variety of patients in order to meet the patients' health care needs as is typically acquired through the completion of an Associate's or Bachelor's degree in Nursing from an accredited school. Bachelor's degree in Nursing preferred.
  2. Must hold a current active Nurse license from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act. 
  3. Minimum of two years of relevant nursing experience required. 
  4. Utilization review and social work experience preferred. 
  5. Strong analytical, communication, and problem-solving skills. Ability to effectively communicate with individuals from diverse backgrounds. 
  6. Ability to maintain strict confidentiality with regard to protected and sensitive information.
  7. Ability to complete reports and correspondence at a professional level. 
  8. Ability to immediately respond to common inquires and complaints from patients, employees, and regulatory agencies in collaboration with senior leadership. 
  9. Successful completion of required knowledge and training of standard precaution and transmission protocols and when to apply during principle duties and responsibilities.  


Working Conditions


  1. Works in a normal office work environment with little exposure to excessive noise, dust, temperature etc.  
  2. Sitting, walking, and/or standing for up to 90% of work time when doing Utilization Review, Preauthorizing Orthopedic and Urology surgeries and Mental Health Assessments, etc.  Up to 20% of work time may push items weighing up to 50 pounds when assisting nursing staff if needed. 
  3. Comes in contact with a variety of isolation patients (up to 5% of work time), but seldom severe hazards. Follows the appropriate safety education given to prevent any at risk situations or environments.  
  4. Spends up to 40% of day seated, completing paper/computer work. The rest of the 60% percent of the day is spent completing various patient-related activities, i.e. meeting with patients and caregivers. May assist during emergency situations to help transfer patients, requiring 35 pounds to pivot transfer a patient. May assist in transferring patient beds down halls, up to 35 pounds of horizontal force pounds to move bed up to 200 feet in distance. May also assist with bed and chair mobility to help assist patient into a better position. May take part in 1-2 meetings per day, whether in hospital, Emergency Room or Family Medicine Specialists clinic. Will need to ambulate around the hospital up to 30% of day to 700 feet to Family Medicine Specialists clinic. Stocks own shelves with information, lifting up to 7 pounds approximately 50 inches in height to 3 inches from floor. A cabinet contains overhead storage that may need accessed with materials 1-2 pounds in weight. 
  5. Exposure to chemicals and other hazards related to patient care. Appropriate safety precautions, such as safety glasses, gloves, etc. must be used to minimize risk of injury. 
  6. Direct exposure to body substances during patient care activities and on contaminated surfaces such as patient equipment.
