Lexington Medical Lodge is a 128 bed facility focused on short-term rehabilitation and long-term care. We are looking for a Social Worker to manage patient advocacy, discharge planning, and family coordination. Our facility operates with a focus on clinical results and clear resident outcomes. You will work closely with our admissions and nursing teams to ensure every resident has a clear path from intake to a safe discharge.
Pay, Schedule and Benefits
- Pay Rate: Starting at $30/hour.
- Schedule: Monday - Friday, 8am-5pm (Occasional flexibility required based on resident needs).

Qualifications
- Valid Social Work license in the State of Texas (LBSW or LMSW).
- Previous experience in a skilled nursing or long-term care setting.
- Knowledge of Medicaid, Medicare, and private insurance protocols.
Skills
- Ability to manage complex discharge planning for short-term rehab patients.
- Strong verbal communication for family consultations and care conferences.
- Proficiency in electronic medical record (EMR) documentation.
Responsibilities
- Coordinate all resident admissions, transfers, and discharges.
- Conduct initial psychosocial assessments and annual updates for all residents.
- Lead care plan meetings with families and the interdisciplinary team.
- Identify and provide medically related social services to maintain resident well-being.
- Assist residents with financial applications and community resource referrals.
