Long Term Sub School Adjustment Counselor

The School Social Worker/School Adjustment Counselor is a critical member of the Leahy Middle School student support system. The position provides direct social-emotional and behavioral support while coordinating comprehensive interventions for students experiencing significant academic, social-emotional, behavioral, attendance, or family-related challenges. The School Social Worker/School Adjustment Counselor works collaboratively with students, families, teachers, administrators, counselors, special educators, interventionists, attendance personnel, mental health providers, community agencies, and other partners to identify student needs, develop individualized support plans, coordinate interventions, monitor progress, and ensure continuity of care. The position plays an important role in the school's MTSS/RTI and Student Support systems, using information from multiple sources to identify student needs, coordinate appropriate interventions, monitor outcomes, and support teams in making data-informed decisions about student supports. This is a highly collaborative role for an educator who sees student support as both relational and systemic. The successful candidate will build strong relationships with students while also helping teams understand student needs, coordinate interventions, monitor progress, and strengthen systems of support across the school.
