Case Manager (CCSP Medicaid) - Hybrid/Field-Based | Metro Atlanta, GA
LegacyPointe Care Management is seeking a Case Manager to support our growing CCSP (Community Care Services Program - Medicaid) client population across Metro Atlanta.
This role is ideal for professionals with a background in social services or healthcare who are organized, responsive, and able to manage a caseload independently while coordinating care in a structured, fast-paced environment.
Compensation
- Base Pay: $2200.00 - $2800.00 per month (based on experience and caseload)
- Per Client: $30-$35 per active client, per month
- Mileage reimbursement provided for required field visits

Responsibilities
- Manage assigned CCSP (Medicaid) caseload
- Complete required client contacts and follow-ups
- Coordinates services with providers, families, and community resources
- Monitor client status and respond to changes in condition or needs
- Maintain accurate, timely, and compliant documentation
- Ensure adherence to CCSP program requirements and company policy and timelines
Requirements
- Bachelor's degree in Social Work, Human Services, or related field
- Experience in case management, Medicaid programs, or social services
- Strong organizational and communication skills
- Ability to manage multiple clients and priorities independently
- Reliable transportation for required in-home visits
