What You Bring to the Table:
We are seeking a dedicated Outpatient Physical Therapist with a Doctor of Physical Therapy (DPT) degree from an accredited program and current state licensure (or eligibility). The ideal candidate is passionate about patient care, skilled in developing individualized treatment plans, and comfortable working collaboratively in a team-oriented outpatient environment. Experience in outpatient settings is preferred but not required.
Key Responsibilities:
- Conduct comprehensive patient evaluations, including assessments of physical condition, mobility, strength, and functional abilities
- Develop individualized treatment plans based on patient goals and evidence-based practices
- Implement therapeutic interventions such as manual therapy, therapeutic exercises, modalities, and patient education
- Maintain accurate and timely documentation of evaluations, treatments, progress notes, and outcomes using Raintree
- Educate patients and their families on home exercise programs, rehabilitation processes, and preventative care strategies
- Monitor patient progress, regularly reassess treatment plans, and make adjustments as needed to optimize outcomes
