Legacy Personnel is seeking an experienced Clinical Social Worker for a 13-week contract assignment in Albany, Oregon. This position supports hospice patients and their families in home-based settings and offers a consistent weekday schedule with no weekends.

Responsibilities
- Provide medical social work services to hospice patients and their families
- Support patients through the final stages of illness
- Provide psychosocial assessments, counseling, and emotional support
- Assist patients and families with care planning and community resources
- Collaborate with nurses, physicians, hospice aides, spiritual support, and other interdisciplinary team members
- Provide support in home settings, skilled nursing facilities, assisted living facilities, and adult foster homes
- Assist with bereavement and end-of-life support
Requirements
- Minimum 1 Year of Social Work Experience
- Oregon LMSW or CSWA License Required
- BLS Certification Required
- Oregon LCSW preferred
- Previous hospice experience preferred
- Hospital or medical social work experience preferred
- Valid Driver's License required
- Reliable vehicle maintained in good working condition
Preference will be given to candidates with previous hospice experience.
