As a Case Manager I, you will coordinate supportive services, assist with housing placement, and provide ongoing guidance to individuals navigating complex life challenges. Duties
- Conduct outreach, intake, and comprehensive assessments to identify participant needs related to housing and supportive services.
- Develop, review, and update Individual Service Plans to address barriers and promote self-sufficiency.
- Assist participants with securing housing, including apartment searches, landlord communication, lease review, and move-in coordination.
- Support participants with obtaining utilities, paying deposits, and maintaining housing stability.
- Conduct home visits to ensure safe, stable living environments and program compliance.
- Provide peer support, life skills training, and relapse prevention support as appropriate.
- Connect participants to community resources, including healthcare, mental health, employment, and benefits.
- Accompany participants to appointments and assist with applications and follow-up services.
- Maintain accurate and timely documentation, including progress notes, assessments, and reports.
- Complete data entry and reporting requirements in accordance with program standards.
- Assist in crisis intervention and connect participants to emergency or mental health services when needed.
- Conduct alcohol and/or drug screenings as appropriate.
- Serve as a liaison between Legacy III and community partners, landlords, and service providers.
- Attend agency and community meetings to advocate for participants and support program goals.
- Maintain strict confidentiality of participant and agency information.
- Perform other duties as assigned by the Director of Programs.
Requirements
- Minimum of an Associate degree in Social Work, Chemical Dependency, or a related field (Bachelor’s and Master’s degrees are accepted and encouraged).
- Minimum of one (1) year of experience working with individuals experiencing homelessness, substance use, mental health challenges, or related populations
- Strong communication, documentation, and organizational skills.
- Ability to assess needs, solve practical problems, and manage multiple responsibilities.
- Experience with computer systems and data entry (e.g., Microsoft Office and database systems)
- Must possess a valid Ohio driver’s license and maintain auto insurance.
Pay: $18.00 - $22.00 per hour
