Coleman Health Services, a nationally recognized nonprofit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a Case Manager to provide case management and wraparound services to Summit County adults and families located in Akron, Ohio.
The Case Manager will work collaboratively with clients, families, treatment teams, community providers, and agencies including Probation, Parole, Courts, Social Security, DJFS, and other community partners to support successful transitions and continuity of care.

Job Duties:
- Provide short-term case management and wraparound services
- Connect clients and families with community resources and ongoing care
- Support transitions to appropriate services
- Collaborate with internal and external treatment teams and community partners
Minimum Qualifications:
- Bachelor's degree in a social service-related field
- One year of related experience preferred
- Knowledge of community resources and social service systems
- Willingness to obtain an NPI and Medicaid billing number (Coleman will provide assistance)
- Maintain an excellent driving record
- Ability to work collaboratively with clients, families, treatment teams, and community partners
