Responsibilities:
- Conduct hospice eligibility assessments based on thorough knowledge of Medicare Hospice Regulations.
- Educate patients, families, and facility staff about hospice services and patient-specific care plans.
- Act as a primary point of contact to ensure coordination and continuity of care.
- Utilize strong communication and time management skills to effectively manage multiple cases and priorities.
- Participate in interdisciplinary team meetings to review patient plans and compliance with hospice criteria.
- Provide ongoing support and education on the hospice process to healthcare providers within the community
Nurses already doing similar transition-heavy work:
- Hospital case managers / discharge planners
- ICU, ER, oncology, and cardiac nurses (high exposure to end-of-life care)
- Palliative care nurses
- Home health nurses ready for a more relationship-based role
Qualifications:
- Must possess an active RN license.
- Proven hospice experience with a deep understanding of Medicare regulations and eligibility requirements for hospice care.
- Exceptional interpersonal, communication, and presentation skills.
- Robust organizational and time management skills are essential.
- Must be adaptable, patient-focused, and capable of handling challenging situations with tact and empathy.
