This full-time Visiting Nurse (RN) role is ideal for nurses looking to step away from the hospital or clinic setting and into a more autonomous, relationship-focused position. You’ll conduct skilled nursing visits to clients in the comfort of their homes, typically seeing 6–7 clients per day in a designated and strategic territory. In addition to your primary territory, this role includes occasional floating to nearby territories to help support patient load fluctuations—offering variety and an opportunity to make an even greater impact across our service area.

What You’ll Do:
- Conduct nursing assessments and develop/update care plans
- Provide wound care, medication management, and patient education
- Collaborate with RN Case Managers, Supervisors, and the broader Care Team
- Educate and support patients and family members to promote independence
- Occasionally float to nearby territories based on census needs
What You’ll Need:
- Active RN license in the State of Minnesota
- Clinical nursing skills applicable to a wide range of medical needs
- Reliable transportation for home visits across the metro area
- Basic computer literacy and willingness to learn user-friendly EMR systems
