Role Overview: PRN Nurse Practitioner - Richmond, VAWe are seeking a dedicated and compassionate Nurse Practitioner to join our Richmond team on a PRN basis. This role provides an opportunity to deliver high‑quality, patient‑centered care in post‑acute settings, including long‑term care and assisted living facilities, with flexible scheduling to accommodate clinician availability.

Provide comprehensive, evidence‑based medical care within post‑acute and long‑term care settings on an as‑needed (PRN) basis
Perform timely assessments, develop and implement person‑centered treatment plans, and manage ongoing care
Prescribe and manage medications within scope of practice and in accordance with Virginia regulations; conduct medication reconciliation and monitoring
Collaborate with nursing staff, facility leadership and interdisciplinary teams to support resident safety, transitions of care and positive outcomes
Document patient encounters accurately and promptly in the Electronic Medical Record (EMR), including clinical reasoning, risk plans and follow‑up
Provide education and support to residents, families and facility staff regarding care plans, medication effects and strategies for safe, dignified care
Participate in case reviews, care conferences and quality improvement activities as requested
Master’s or Doctoral degree in Nurse Practitioner studies from an accredited institution
Active, unrestricted Nurse Practitioner license in Virginia and current DEA registration where required for prescribing
Proven proficiency with Electronic Medical Record systems and timely documentation practices
Excellent clinical judgement, therapeutic communication skills and the ability to work autonomously while collaborating within teams
Experience in post‑acute care, long‑term care, geriatrics or similar settings preferred

Schedule
This is a PRN position with flexible scheduling based on facility coverage needs and mutual agreement. Assignments will typically include scheduled daytime on‑site visits and occasional urgent coverage depending on demand.
