- Provide comprehensive, evidence‑based medical care within post‑acute and long‑term care settings on an as‑needed (PRN) basis
- Perform timely assessments, develop and implement person‑centered treatment plans, and manage ongoing care
- Prescribe and manage medications within scope of practice and in accordance with Virginia regulations; conduct medication reconciliation and monitoring
- Collaborate with nursing staff, facility leadership and interdisciplinary teams to support resident safety, transitions of care and positive outcomes
- Document patient encounters accurately and promptly in the Electronic Medical Record (EMR), including clinical reasoning, risk plans and follow‑up
- Provide education and support to residents, families and facility staff regarding care plans, medication effects and strategies for safe, dignified care
- Participate in case reviews, care conferences and quality improvement activities as requested
- Master’s or Doctoral degree in Nurse Practitioner studies from an accredited institution
- Active, unrestricted Nurse Practitioner license in Virginia and current DEA registration where required for prescribing
- Proven proficiency with Electronic Medical Record systems and timely documentation practices
- Excellent clinical judgement, therapeutic communication skills and the ability to work autonomously while collaborating within teams
- Experience in post‑acute care, long‑term care, geriatrics or similar settings preferred
- Timely, comprehensive assessments and person‑centered interventions that support resident safety and wellbeing
- Clear, accurate EMR documentation and effective communication with facility staff, residents and families
- Strong, collaborative relationships with facility leadership and interdisciplinary teams
- Positive contributions to quality improvement activities and resident care outcomes
