Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide therapy services in accordance with physician orders
- Document evaluations, treatments, progress notes, and discharge summaries
- Collaborate with nursing and interdisciplinary teams
- Participate in care planning and utilization review meetings
- Educate residents, families, and staff on therapy goals and plans
- Maintain productivity and documentation standards
- Support regulatory compliance and survey readiness
- Graduate of an accredited therapy program (SLP)
- Current state licensure in SLP
- Current Certification of Clinical Competence (CCC)
- Long-term care experience preferred
- Strong documentation and communication skills
- Patient-focused and team-oriented
- Standing, walking, bending, and assisting residents
- Ability to lift and support residents during therapy
- Exposure to clinical environments
