Responsibilities
- Coordinate and administer physician-ordered wound care and treatments
- Perform skin assessments on all new admissions and ongoing residents
- Monitor, photograph, and document wound progress and interventions
- Communicate changes in wound condition to physicians, nursing leadership, and families as appropriate
- Maintain accurate and timely wound documentation and treatment records
- Participate in care planning, MDS support, and interdisciplinary collaboration related to wound care
- Assist with tube feedings and related treatments as assigned
- Order wound care supplies and maintain appropriate inventory levels
- Participate in quality assurance, survey readiness, and audits related to skin integrity
- Provide education and guidance to nursing staff on wound care practices
- Complete incident reports and documentation related to skin or treatment concerns
- Ensure treatment areas are clean, safe, and compliant with infection control standards
- Current, active RN or LPN license in the state
- Wound care or treatment nursing experience preferred
- Strong knowledge of skin integrity, wound staging, and treatment protocols
- Excellent documentation and organizational skills
- Ability to work independently and meet regulatory timelines
- Strong communication and interdisciplinary collaboration skills
- Standing and walking throughout the facility
- Bending, lifting, and assisting residents with positioning and treatments
- Exposure to clinical environments, bodily fluids, and wound care supplies
