Responsibilities
- Coordinate the admissions process from initial referral through resident admission
- Review referrals to ensure medical appropriateness and insurance viability
- Communicate with hospitals, discharge planners, residents, and families
- Complete and verify all admissions paperwork and required documentation
- Explain facility services, payer sources, and financial information to residents and families
- Coordinate with Nursing, Social Services, and Business Office to support smooth admissions
- Schedule and conduct facility tours as needed
- Update census boards and admission tracking tools daily
- Assist with discharge planning and follow-up as assigned
- Maintain strong working relationships with referral sources and community partners
- Participate in surveys, audits, and admissions-related quality activities
- Maintain confidentiality and comply with HIPAA requirements
- High school diploma required; bachelor’s degree preferred
- Prior admissions or healthcare experience preferred
- Long-term care or hospital discharge planning experience strongly preferred
- Knowledge of Medicare, Medicaid, and payer sources preferred
- Strong communication, organization, and follow-up skills
- Professional, dependable, and resident-focused
- Office and facility-based work throughout the day
- Standing, walking, and touring the facility as needed
- Frequent interaction with residents, families, and staff
