Day Shift - 7.5 Hours (United States of America) - Provide culturally proficient education and support to patients eligible for cancer screening. Ensure understanding of steps needed to complete cancer screening process. - Recruit potential cancer screening participants from priority populations through mailings, phone contact, and or face-to-face contact. - Perform case management for abnormal screenings results and monitor timely and appropriate specialty care services. - Work with community resources to help patients access support systems and help them move through the complexities of the health care system. - Share information acquired through conferences and trainings sessions with other agency members and with community members as appropriate in order to reduce duplicate efforts. - Participates in outreach events, keeping track of participants, tests performed and results. Reports information of outcomes to patients, designated physicians and leadership. - Maintain up-to-date information systems for procession bill and case evaluations. - Performs quality audits as requested. Monthly statistics related to outreach, screening, and trainings events. Address barriers to successful outcomes of events. - Performs other related duties as assigned, including but not limited to, clerical and community outreach event participation. - Distribute and instruct use of FIT kits. Following up on returns via phone calls to ensure 90% compliance with returns.
