📅 Schedule: Monday-Friday 8:00a-4:30pm, every other weekend on-call rotation, holidays, evenings and weekends as needed

💰 Starting Salary: $65,788–$72,366.80/year (DOE)
📅 Schedule: Monday-Friday 8:00a-4:30pm, every other weekend on-call rotation, holidays, evenings and weekends as needed
✨ What You’ll Do
- Lead and oversee all functions of the Social Services Department in our Health Care Center.
- Supervise and mentor a compassionate team, ensuring top-quality care and compliance with state and federal regulations.
- Provide direct social work services to residents, guiding care plans and psychosocial support.
- Coordinate discharge planning and build partnerships with community agencies.
- Advocate for resident rights and emotional well-being.
- Conduct audits, develop programs, and drive quality improvement initiatives.
- Collaborate closely with nursing, therapy, chaplaincy, and other departments to deliver holistic care.
- Bachelor’s or Master’s in Social Work or related field (required)
- 3 years’ experience in long-term care, skilled nursing, or healthcare (required)
- 1 year of supervisory experience preferred
- Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) preferred
- Strong leadership, communication, and organizational skills
