Qualifications
- Knowledge and experience in case management, client assessment, and care planning
- Strong interpersonal, communication, and advocacy skills to build relationships with clients and stakeholders
- Understanding of and ability to address healthcare disparities and social issues impacting the Latino community
- Proficiency in maintaining accurate and organized documentation
- Bilingual in English and Spanish is strongly preferred
- Ability to work collaboratively and independently in an on-site team
- High school diploma
- Previous experience in social services, healthcare, or community outreach is a plus
