Patient Care Coordinator

LaSante Health Center is seeking an experienced Patient Care Coordinator to support patients in accessing the care they need. This role involves managing appointment scheduling, insurance verification, intake documentation, and vendor coordination while providing excellent service to patients and families.

Responsibilities:

• Schedule appointments for new and returning patients
• Create and update patient profiles in the electronic health record (EHR)
• Verify insurance coverage and patient eligibility
• Ensure completion of required forms and documentation
• Answer patient inquiries in person and over the phone
• Coordinate with external vendors such as labs, transportation providers, and interpreters
• Maintain HIPAA compliance and patient confidentiality
• Assist the front desk and administrative team with daily operations

• Previous experience in a medical office or healthcare setting required
• Knowledge of insurance verification and patient intake processes
• Proficiency with EHR systems
