Responsibilities
- Assess client needs and develop comprehensive care plans
- Provide case management and monitor ongoing services
- Coordinate referrals and connect clients with appropriate resources
- Maintain up-to-date client files and documentation
- Ensure compliance with program requirements and timelines
- Collaborate with internal teams and community partners
- Travel to client locations within assigned areas
- Participate in trainings and maintain required certifications
- Support continuous service improvement initiatives
- Communicate effectively with clients, families and stakeholders
- Bachelor’s degree in a related field required
- Strong communication and interpersonal skills
- Ability to manage multiple clients and priorities effectively
- Experience with data entry and case management systems
- Critical thinking and problem-solving skills
- Valid driver’s license and proof of insurance required
- Willingness to travel locally for client visits
- IDOA Certification required upon hire
- Experience in healthcare, social services or case management preferred
- Compensation: $18.90 – $21.00 per hour, depending on experience
