What are the responsibilities and job description for the QIDP position at LARC?
ESSENTIAL JOB FUNCTIONS:
Provides the Individual Habilitation Plan or Personal Center Plan for clients of the Client Development Department while collaborating with the Interdisciplinary Team on the development and implementation of such plans.
· Responsible for all aspects of programming and development of assigned clients.
· Responsible for ensuring that the client's programs meet State Licensing, OCDD and Waiver Standards.
· Must chair the Individual Habilitation, ICEP, Personal Centered and Interdisciplinary Team Meetings and write goals for the above mentioned plans.
· Responsible for the assessment and goal selection process for clients' IHP, PCP, CPOC's and ICEP's, as required. Assess client development and provide written training programs in vocational, self-help, community awareness, socialization, etc., to meet identified client's needs.
· Assists in ensuring that behavior management plans are in place for applicable clients. Train staff on the implementation of those plans.
· Ensure that training and behavioral modification programs are consistent with each client's plan.
· Must review IHP, PCP and/or ICEP documentation quarterly, monthly for waiver clients monitoring data for accuracy, and prepare quarterly reviews.
· Responsible for the submission of those reports to parents, guardians, case managers and/or appropriate agencies.
· Ensure that all professional reviews are received in a timely fashion, disseminated to appropriate parties, agencies and appropriately filed. Re-call I.D. team to revise IHP, PCP and/or ICEP when deemed necessary.
· Responsible for the discharge summaries of assigned clients.
· When necessary, investigate case situations and present information to the Director of Client Development, Interdisciplinary Team, client's family, case managers and when necessary the Human Rights Committee.
· Help client to respond constructively to Individual Habilitation Plan, Personal Center Plan and/or ICEP and assist in adjustments needed to obtain the goals set in those plans.
· Render explanation of the Individual Habilitation Plan, ICEP, Personal Center Plan, Vocational Sector of the CPOC and/or ICEP to the client and the client's family.
· Monitor staff and clients to ensure that Individual Habilitation Plan, Personal Center Plan, CPOC, or ICEP's are being implemented as outlined in those plans.
· Serves as link between client, instructor, client's family, other agencies, and community.
· Refers client or client's family to other community resources.
· Must be a participant of the Vocational/Residential Admissions Committee.
· Responsible for maintaining current information on file for assigned clients.
· Must coordinate the preparation of progress reports from appropriate service areas as required (Supported Work Systems).
· Attend schedule staff meetings.
· Protect all clients from any form of Abuse and/or Neglect.
· Responsible for the completion of client incident reports, distribute such reports to client's family and/or group home and case manager within 24 hours, ensuring that the Director of Client Development reviews each incident report.
· Write memos to communicate needed information to staff and/or other responsible parties.
· Responsible for knowing and carrying out the established policies and procedures of the LARC and State Licensing and BCSS.
· Maintain clients and employee rights and confidentiality at all times.
· Required to attend on-going trainings.
· Performs other routine duties as assigned by the Director.
