Job Summary:
We are seeking a Residency-Trained Clinical Pharmacist with strong experience in an acute/inpatient setting. The candidate will be responsible for ensuring safe and effective medication use, supporting clinical teams, and providing high-quality patient care in a hospital environment.
- Weekdays with variable weekend requirements
- Start Date: June (preferred)
- Review, verify, and dispense medications accurately
- Provide clinical pharmacotherapy support to healthcare teams
- Monitor drug therapy for effectiveness, interactions, and adverse effects
- Perform dosing adjustments (renal, cardiac, anticoagulation, etc.)
- Prepare and verify IV medications, including sterile compounding and chemotherapy (if applicable)
- Participate in therapeutic drug monitoring and pharmacokinetic dosing
- Manage high-alert medications and ensure patient safety
- Collaborate with physicians, nurses, and interdisciplinary teams
- Assist in medication storage, inventory, and drug shortage management
- Utilize automated dispensing systems (e.g., Pyxis, Omnicell)
- Ensure compliance with hospital policies and regulatory standards
- Residency-Trained Pharmacist (Required)
- Acute / Inpatient Pharmacy experience (Required)
- PharmD or equivalent degree
- Active Pharmacist License (State-specific)
- Strong knowledge of clinical pharmacotherapy and hospital workflows
- BCPS Certification (Preferred)
- BLS Certification
- IV Certification / Sterile Compounding experience
- Experience with chemotherapy or hazardous drugs
- Familiarity with medication dispensing systems (Pyxis, McKesson, etc.)
- IV preparation (antibiotics, TPN, IV push, chemotherapy)
- Clinical dosing (renal, pediatric, geriatric, anticoagulation)
- Pharmacokinetics and therapeutic drug monitoring
- Management of a wide range of disease states (cardiovascular, infectious disease, oncology, etc.)
- Strong communication and teamwork skills
- Acute Care Hospital (Inpatient)
- May involve collaboration with outpatient/ambulatory or infusion services
