About the Role
Join a collaborative clinical team providing on-site care in skilled nursing facilities. This role focuses on delivering high-quality, geriatric-centered care, managing both acute and chronic conditions, and working closely with interdisciplinary teams.
Key Responsibilities
- Perform comprehensive patient assessments, including histories and physical exams
- Diagnose and manage acute and chronic conditions using evidence-based guidelines
- Order, interpret, and document diagnostic tests and clinical findings
- Develop and implement individualized treatment plans
- Prescribe medications and therapies within scope of practice
- Coordinate referrals and collaborate with physicians and care teams
- Educate patients and families on treatment plans and preventive care
- Maintain accurate, timely clinical documentation
Qualifications
- Licensed Nurse Practitioner (NP) (FNP, AGNP, or related specialty)
- Active state license and prescriptive authority
- DEA license (or ability to obtain)
- Strong clinical judgment and communication skills
- Experience in geriatrics or post-acute/SNF settings preferred
- New graduates are welcome

Salary : $120,000 - $150,000
