We are actively seeking an experienced Nurse Practitioner (NP) to join a collaborative, community-focused healthcare team. This role is centered on delivering comprehensive, patient-centered primary care within an integrated model that includes behavioral health and wellness services.
Position Overview
The ideal candidate is comfortable managing diverse patient populations, addressing both acute and chronic conditions, and working within a multidisciplinary care environment.
Key Responsibilities:
• Provide comprehensive primary care services, including assessment, diagnosis, and treatment planning
• Manage acute and chronic conditions across a diverse patient population
• Prescribe and manage medications within scope of practice
• Perform physical examinations and review medical histories
• Deliver preventive care and health maintenance services
• Educate patients on disease management, medications, and lifestyle modifications
• Collaborate with interdisciplinary teams including behavioral health and care coordination staff
• Refer patients to specialists as appropriate
• Participate in after-hours on-call rotation
• Maintain accurate and timely clinical documentation
• Engage in team meetings, peer reviews, and quality improvement initiatives
• Provide coverage support for other clinicians as needed
Qualifications:
Required
• Master’s degree or higher as a Nurse Practitioner
• Active California NP license
• Current DEA registration
• Minimum 2 years of clinical experience as an NP
• Background in Family Medicine, Internal Medicine, or Primary Care
Preferred
• Experience in community health, FQHC, or integrated care settings
• Familiarity with behavioral health integration
• Strong clinical judgment and communication skills
• Ability to work effectively within a multidisciplinary team
• Bilingual proficiency preferred: Spanish (highly preferred), Mandarin, Cantonese, Tagalog, Farsi, or Vietnamese
