Job Summary
The Case Manager working in ABH will support clients to develop executive functioning, organizational, employment, emotional, and self-advocacy skills required to meet their basic needs and develop lives worth living. The Case Manager will support clients to understand their diagnoses and make thoughtful decisions about how to manage symptoms and impacts of mental health concerns. The Case Manager will collaborate closely with team members across settings to share clinical observations, prioritize client autonomy and self-determination, and coordinate care.
The Case Manager provides case management, psychoeducation, and clinical recommendations to a caseload. The Case Manager works with clients to develop treatment plans and implement therapeutic interventions to support clients in reaching established goals across settings, including in homes, community, and the agency. The Case Manager coordinates services, including making referrals to additional services when needed (including mental health crises), collaborates with other service providers, maintains appropriate documentation of services provided, and may provide transportation for clients. The Case Manager follows established guidelines of ethical practice in accordance with Vermont State Law.

Education/Experience
Required: Bachelor’s degree in process or HS Diploma with minimum of 3 years of relevant experience
