Job Overview
We are seeking compassionate and dedicated Home Health Aides to provide high-quality care to patients in their homes and community settings. The ideal candidate will have experience in patient observation, assisting with Activities of Daily Living (ADLs), and working with diverse populations including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's or dementia care. This role offers an opportunity to make a meaningful difference in patients' lives by delivering personalized, respectful, and professional home health services.
Responsibilities
- Assist patients with ADLs such as bathing, dressing, grooming, and mobility support
- Monitor and document patient condition, vital signs, and medical records accurately using EMR/EHR systems
- Administer medications following prescribed instructions and maintain medication records
- Support patients with medical procedures including catheter care, Hoyer lift transfers, and other clinical tasks
- Provide companionship and emotional support to enhance patient well-being
- Assist with meal preparation and light housekeeping tasks including laundry and cleaning to maintain a safe environment
- Follow HIPAA regulations to ensure patient confidentiality at all times
- Recognize and respond appropriately to patient needs, including emergencies or changes in condition
- Support patients with memory care needs such as Alzheimer's or dementia through specialized techniques
- Collaborate with healthcare teams to ensure comprehensive patient care plans are followed
Requirements
- Prior experience in home health, assisted living, nursing homes, or long-term care preferred
- Knowledge of medical terminology, medication administration, and medical documentation procedures
- Certification in CPR is required; additional certifications such as hospice care or hospice training are a plus
- Experience working with people with developmental disabilities or disabilities is highly valued
- Ability to work independently while adhering to care plans and safety protocols
- Strong communication skills for effective interaction with patients, families, and healthcare professionals
- Valid driver’s license preferred for travel between patient locations
- Ability to pass background checks and comply with HIPAA regulations
- Compassionate attitude with a genuine desire to improve patient quality of life

Pay: $16.00 - $21.00 per hour
