Position Overvie

wLakeshore Talent is partnering with a mission-driven organization dedicated to supporting individuals with disabilities and those in need of long-term care services in their search for an Intake Case Manager. The Intake Case Manager plays a vital role in helping individuals access essential supports by conducting functional assessments and guiding them through eligibility and application processes

.This position requires a balance of empathy, accuracy, and compliance, ensuring individuals receive timely and appropriate services while meeting all regulatory requirements

: Greater Colorado Springs Area, hybrid schedule (in-office, remote, and in community settin

Qualificat

  • nts:Bachelor’s degree in social services or similar stud
  • y OR5 years of experience working with vulnerable populations, individuals with disabilities, or within human services/non-profit setting
