wLakeshore Talent is partnering with a mission-driven organization dedicated to supporting individuals with disabilities and those in need of long-term care services in their search for an Intake Case Manager. The Intake Case Manager plays a vital role in helping individuals access essential supports by conducting functional assessments and guiding them through eligibility and application processes
.This position requires a balance of empathy, accuracy, and compliance, ensuring individuals receive timely and appropriate services while meeting all regulatory requirements
