Position Summary


The Patient Navigator is a hybrid role that provides personalized coordination of healthcare services for patients in the Lakeland Regional Health Graduate Medical Education ambulatory clinics. The Patient Navigator works collaboratively with patients and caregivers, healthcare team members, and community organizations to identify barriers to optimal care outcomes. The Patient Navigator develops and implements strategies to drive patient engagement, improve adherence with treatment plans, lower barriers to care, and improve healthcare and cost outcomes. Patient navigator primary focuses includes the functions of registration, scheduling, authorizations, referrals, appointment follow-ups, financial screenings, patient assistance and the provision of information/assistance with community resources.

    • Standard Work: Patient Navigator
    • Provides navigation services including financial screening, authorization, referral to appropriate programs and services. This includes the screening of uninsured patients to see if they qualify for governmental funding such as Medicaid or Polk Healthcare Plan. If patient does not qualify for an available governmental program, the Patient Navigator then provides navigation to complete the application process for one of LRH’s community programs.
    • Follow set guidelines for verifying identity, correctly capturing, recording, and updating all required data for every patient.
    • Maintains knowledge base of all programs offered by LRH for Charity and Financial Assistance and the requirements to qualify.
    • Monitors patient eligibility for LRH community programs for yearly renewals, including providing patient outreach to complete renewal timely.
    • Navigates patients that are uninsured or underinsured to appropriate available Medication Assistance Programs, including timely renewals as appropriate.
    • Assists eligible patients in navigating the Welldyne prescription program.
    • Timely inputs all services of LRH Community programs into Famcare for payment of services.
    • Maintains all EPIC Morrell Internal Medicine and Kathleen Family Medicine work queues, ensuring that all entries are completed within three days of receipt.
    • Provides navigation services for the Hospital Business Office requests from walk-in patients. (ie: Payroll Deductions; itemized statements; payment plans; billing questions and complaints).
    • Coordinates surgical appointments and procedures as ordered by the provider, in an ambulatory, hospital or outpatient surgical center.
    • Works with external/internal providers/patients to obtain any necessary referrals/authorizations/medical records to support diagnosis in order to meet department scheduling guidelines.
    • Works closely with referral partners (Polk Health Plan, We Care of Central Florida, Moffit, etc.) to ensure all documentation has been received (via scanning or faxing of documentation).
    • As Patient needs are expressed or identified, provides guidance to beneficial community resources and coordinates follow-up appointments/referrals as needed. Returns phone messages within 24 business hours.

  • Interacts professionally with others and communicate with compassion and courtesy at all times. Establishes close relationships with and serves as primary point of contact for clients.


Qualifications & Experience


Essential:

High School or Equivalent

Essential:

2 years of patient accounting experience


Other information:
EXPERIENCE ESSENTIAL:
  • 2 years of patient accounting experience
  • Knowledge of medical insurance (Medicare, HMO's, PPO's, Commercial), hospital billing, coding (ICD-10) and collection process.
  • One year of healthcare experience in a clinical environment

EXPERIENCE PREFERRED:
  • Knowledge of authorizations process and surgery scheduling procedures.
  • Strong working knowledge of the function and use of electronic health records.
  • Experience in Community Case Management
  • Bilingual (English/Spanish)

LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED:
  • Graduate of an accredited Medical Assisting training program
