Position: RN Case Manager / Visit Nurse
Schedule:
- .8 FTE
- 8:00am–4:30pm
- Weekend On-Call Rotation (may vary by need)
- Flexible scheduling competitive on-call pay

What You’ll Do:
- Visit patients in their home setting—wherever they call home
- Deliver compassionate, patient-centered hospice care
- Manage symptoms, educate families, and provide holistic support

Current Michigan RN License required at time of hire.
