Lake Pointe Rehab is looking for an outgoing, compassionate, upbeat candidate for our Social Services Designee.
Social Services Designee has many different tasks associated with the role here at Lake Pointe including: MDS, social service assessments in Point Click Care, ancillary services for all residents, discharge planning, equipment ordering for discharge, and any social service concerns that may arise for each and every resident. Must be able to be organized and follow direction of administrator and handle concerns appropriately.
