Job Summary
We are seeking a dedicated and compassionate Physical Therapy Assistant (PTA) to join our Lagniappe Homecare team in Natchitoches, La. and covering surrounding communities.
The PTA will work closely with a licensed physical therapist to deliver high-quality rehabilitative care to diverse patient populations in their homes.
This involves implementing individualized treatment plans, assisting with clinical assessments, and ensuring accurate medical documentation.
Key Responsibilities:
- Assist licensed physical therapists in developing and implementing comprehensive treatment plans tailored to patient needs
- Monitor patient progress and document clinical assessments
- Explain procedures to clients while addressing concerns and motivating clients to follow care plans.
- Work with healthcare professionals to coordinate discharge planning and ensure continuity of care
- Maintain detailed medical documentation in compliance with regulatory requirements
Requirements:
- Certification as a Physical Therapy Assistant from an accredited program; valid state licensure or registration required
- Prior experience working in skilled nursing facilities or home health environments preferred
- Experience operating medical equipment used in physical therapy treatments
- Proficiency managing electronic health records (EHR)
- Strong communication skills
- Demonstrated understanding of discharge planning processes and client care coordination
