Responsibilities include:

Assess patient’s nutritional status based on past medical history, diagnosis, interventions and develop care plans for both sub-acute and long term care units
•
Complete nutrition care plans, progress notes and discharge plans in a timely manner
•
Document all initial, annual, quarterly assessments and MDS forms to meet regulatory agency guidelines
