- Perform comprehensive patient assessments and health history documentation using electronic health record (EHR) systems.
- Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or memory care requirements.
- Educate patients and their families on health coaching topics like medication management, nutrition, discharge planning, and chronic disease management.
- Maintain accurate medical documentation adhering to HIPAA regulations.
- Collaborate with multidisciplinary teams in inpatient, outpatient, hospice, or home & community care environments to ensure seamless patient care transitions.
- Proven experience working in diverse healthcare environments including hospitals (ICU, ER), nursing homes, outpatient clinics, or home care.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment.
- Experience caring for individuals with developmental disabilities or those requiring specialized dementia or memory care.
- Excellent communication skills.
- Completing competency assessments on CNA's and PCA's
- Familiarity with infection control protocols and sterile processing techniques.
