Duties
- Conduct patient assessments
- Provide specialized care for diverse populations including pediatrics, geriatrics, neonatal ventilator management, dementia or Alzheimer’s care, hospice & palliative medicine, and working with individuals with developmental disabilities or disabilities.
- Lead discharge planning efforts and case management initiatives to ensure seamless transitions from hospital to home or community-based care settings like assisted living or home care.
- Maintain accurate medical documentation coding for procedures, and detailed medical records in compliance with HIPAA standards.
- Participate in infection control practices and adhere to NCQA standards to promote patient safety and quality improvement initiatives.
Qualifications
- Valid RN license in good standing with current state requirements.
- Proven experience in diverse clinical environments such as hospitals or long-term care facilities.
- Demonstrated proficiency with documentation and patient management.
- Strong knowledgefor accurate patient assessment and diagnostic evaluation.
- Excellent communication skills; compassionate approach when working with children or elderly patients requiring memory or dementia care.
