The Public Health Specialist II is responsible for planning, implementing, administering, and evaluating public health programs and initiatives that support community and population health objectives and disease prevention. Working directly with the public, community partners, and multidisciplinary teams, this position applies established public health principles and analytical methods to deliver programs, collect and analyze data, monitor program activities, support community engagement, and ensure programs are effective, compliant, and responsive to community needs across a variety of settings.

Independently implements and coordinates health education activities using approved materials and established guidance; supports and delivers outreach and presentations; and maintains required documentation in accordance with program procedures and public health policies.
Maintains awareness of relevant services in the region and provides resources, referrals and navigation to other public health programs and services and to outside agencies as appropriate.
Maintains accurate and organized records and related data in accordance with established protocols, and in compliance with applicable laws, regulations and policies.
Maintains the confidentiality, security, and integrity of sensitive and protected public health information and records in accordance with federal, state, and local laws, regulations, and agency policies.

Establishes and maintains effective working relationships with La Plata County Public Health Department (LPCPHD) staff, representatives of other agencies and organizations, community partners, stakeholders, and members of the public.
Collaborates with Communications staff to develop and disseminate program-specific messaging, educational materials, and outreach content.
Participates in meetings, coalitions, workgroups, and community partnerships as assigned.
Independently completes administrative and program support tasks to maintain effective program operations, including managing inventories and supplies, performing financial coding and expense tracking, maintaining program records and databases, and preparing meeting notes and related documentation.
Develops, update, and maintain program materials, tools, and documentation.
Maintain sensitive and confidential client and program information in accordance with HIPAA, departmental policies, and program confidentiality requirements.
Perform assigned duties with professionalism, cultural responsiveness, and adherence to ethical public health standards.
Health Coverage Navigator (HIL)
Serves as a resource for health coverage navigation, supporting individuals and families with complex eligibility, enrollment, or access barriers in accordance with established program guidelines and procedures.
Maintains required certifications as a Certified Application Assistance Site (CAAS) through the Colorado Department of Health Care Policy and Financing and/or as a
Certified Health Coverage Guide through Connect for Health Colorado and applies this knowledge to support program objectives and improve community access to care.
Applies knowledge of health coverage programs and payer options, including Medicaid, CHP , Marketplace, and other resources for uninsured or underinsured individuals, to support informed decision-making, facilitate access to care, and provide appropriate referrals in accordance with established program guidelines.
Maintain up-to-date knowledge of program and system changes, including health insurance coverage and oral health initiatives.
Performs other duties as assigned.
