About the Role:
The Licensed Practical Nurse (LPN) in the Skilled Care Unit plays a critical role in delivering high-quality nursing care to patients requiring specialized and continuous medical attention. This position involves closely monitoring patient health, administering medications, and collaborating with interdisciplinary teams to develop and implement individualized care plans. The LPN ensures patient safety and comfort by performing routine assessments and responding promptly to changes in patient conditions. Additionally, the role requires maintaining accurate and detailed documentation to support continuity of care and compliance with healthcare regulations. Ultimately, the LPN contributes to improving patient outcomes and enhancing the overall quality of care within the skilled nursing environment.
Minimum Qualifications:
- Current and valid Licensed Practical Nurse (LPN) license in the state of employment within the United States.
- Completion of an accredited practical nursing program.
- Basic Life Support (BLS) certification.
- Demonstrated knowledge of nursing principles, patient care techniques, and medication administration.
- Ability to communicate effectively with patients, families, and healthcare team members.
Preferred Qualifications:
- Experience working in a Skilled Care Unit or long-term care facility.
- Certification in geriatric nursing or wound care.
- Familiarity with electronic health record (EHR) systems.
- Additional certifications such as IV therapy or medication administration endorsements.
- Strong organizational and time management skills.
Responsibilities:
- Provide direct nursing care to patients in the Skilled Care Unit, including administering medications and treatments as prescribed.
- Monitor and record patients' vital signs, symptoms, and progress, reporting any significant changes to the supervising Registered Nurse or physician.
- Assist with patient hygiene, mobility, and daily living activities to promote comfort and well-being.
- Collaborate with healthcare team members to develop, implement, and update individualized care plans based on patient needs.
- Maintain accurate and timely documentation of nursing care, patient responses, and any incidents in compliance with facility policies and regulatory standards.
- Educate patients and their families on health management, medication administration, and post-discharge care instructions.
- Ensure adherence to infection control protocols and safety standards within the Skilled Care Unit.
Status - Full-time Days Kremmling, CO
Compensation - $30.00 - $44.00 per hour DOE. $5000 sign-on, $350/mo Housing stipend for FT Perm
