Chatham Glen Healthcare & Rehabilitation Center is seeking a detail-oriented, organized, and dependable Medical Records to join our team! This full-time position plays a vital role in supporting our residents, nursing team, and facility by ensuring accurate, timely, and compliant medical record management.
✨ Position Summary
The Medical Records Clerk is responsible for maintaining, organizing, auditing, and securing all resident medical records in accordance with state and federal regulations. This role supports clinical teams by preparing required documentation, ensuring physician orders are processed and filed, and managing essential recordkeeping processes.
📌 Key Responsibilities
- Maintain and audit active medical charts to ensure accuracy and compliance.
- Close, file, and review medical records for discharged residents; correct deficiencies as needed.
- Scan and file documents in resident charts in a neat, organized, and systematic format.
- Create and maintain assessment schedules within the medical record system.
- Track and file physician orders, signatures, and other required documentation within set timeframes.
- Communicate effectively with internal teams, providers, families, and outside entities as needed.
- Ensure compliance with state/federal regulations, infection control standards, and Residents’ Rights.
- Participate in in-service trainings and QA meetings as required.
- Perform additional duties as assigned within the scope of training and responsibility.

Qualifications

- High school diploma or equivalent required.
- Experience in medical records or healthcare administrative support preferred.
- Knowledge of medical terminology and documentation standards.
- Strong computer skills, including proficiency in Microsoft Word and electronic health record systems.
- Excellent organizational skills and attention to detail.
- Strong verbal and written communication skills.
- Ability to maintain confidentiality and handle sensitive information appropriately.
