Chatham Glen Healthcare and Rehabilitation Center is dedicated to providing exceptional care and compassionate service to our residents. We are seeking a detail-oriented and caring MDS Coordinator to join our team in ensuring compliance with all state and federal regulations.
Position Summary
The MDS Coordinator is responsible for assisting with care planning and completion of MDS assessments and nursing care plans in accordance with state and federal guidelines. This role plays an important part in ensuring quality documentation and resident-centered care.
Responsibilities

Assist in completing MDS 3.0 assessments in a timely and accurate manner.

Support the development and updating of resident care plans as needed.

Participate in care plan meetings with residents, families, and care teams upon admission, significant changes, or quarterly reviews.

Assist with rounds to ensure residents have proper equipment per their care plans (e.g., positioning devices).

Organize, file, and make copies of care plans and documentation.

Answer phones and perform other administrative duties as needed.

Support and uphold all resident rights, confidentiality, and quality improvement initiatives.

Comply with safety procedures, infection control, and all regulatory requirements.

Other duties as assigned by supervisor.
Physical Requirements

Must be able to stand, walk, sit, and use hands frequently throughout the workday.

Occasional lifting up to 50 lbs may be required.

Moderate noise environment typical of an office or healthcare setting.
Requirement
- Active RN or LPN License
- Ability to successfully pass a background screening through the AHCA Clearinghouse
