We are immediately hiring for a FULL TIME Hospice Intake Coordinator
This is an opportunity to work with a committed, creative, fun and fast growing team that is passionate about providing exceptional Community Based Services. Our services are provided in personal homes as well as independent and assisted living facilities. When you join us your key responsibilities will include:
-Position will be cross training and work as Home Health Intake Coordinator as needed
-Support the flow of home health and hospice admissions, promoting efficiency, outstanding customer services, and attention to detail
-Data entry of referrals into the electronic medical record system
-Verify insurance and obtain authorization as needed
-Carefully review medical records to assure appropriate referral information is obtained
-Perform telephonic outreach to physician offices to obtain complete referral information
-Interface directly with patients to support the scheduling of home health and hospice assessments
-Compile and report metrics from the admissions department
-Work collaborative with the Home Health and Hospice Intake Coordinators, covering for these roles as needed
Qualifications:- High school diploma or equivalent
- 1–2 years of experience in healthcare intake, admissions, or a similar administrative role preferred
- Knowledge of home health and hospice services and related terminology (preferred)
- Familiarity with electronic medical record (EMR) systems
- Strong data entry skills with high attention to detail and accuracy
- Ability to verify insurance coverage and obtain authorizations
- Excellent communication skills for interacting with patients, families, and physician offices
- Strong organizational and multitasking abilities in a fast-paced environment
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
- Ability to work collaboratively with team members and cover other intake roles as needed
- Compassionate and professional demeanor when working with patients and families
