The On-Call Hospice Admissions RN provides responsive, high-quality hospice admissions and urgent clinical support during assigned on-call hours. This role is responsible for completing hospice admissions across all levels of care and care settings throughout the King County area, responding to urgent and same-day admissions, and serving as a clinical backup when additional support is required. The RN collaborates closely with Intake, interdisciplinary team members, referral sources, patients, and families to ensure timely, coordinated, and compassionate care.
Schedule & On-Call Expectations
- On-call availability: 9:00 AM – 9:00 PM on designated, set days each week
- Ability to deploy promptly for admissions and visits as needed during assigned on-call hours
Key Responsibilities
Hospice Admissions & Clinical Care
Complete timely hospice admissions for patients qualifying for Routine Home Care or General Inpatient (GIP) level of care across all settings, including hospitals, private residences, assisted living communities, adult family homes, and skilled nursing facilities. Admissions require flexibility, strong clinical judgment, and the ability to collaborate with facility staff, referral sources, and families to support a smooth and compassionate transition into hospice services.
Hospice admissions responsibilities include, but are not limited to:
- Performing comprehensive nursing assessments, including physical, psychosocial, and symptom-focused evaluations
- Reviewing and confirming hospice eligibility in collaboration with Intake and the hospice physician
- Completing medication reconciliation and evaluating current symptom management needs
- Providing patient and family education regarding hospice philosophy, services, benefits, and expectations
- Explaining and obtaining informed consent, election statements, and required hospice admission documentation
- Initiating the hospice plan of care and coordinating immediate needs such as medications, durable medical equipment (DME), and supplies
- Identifying and addressing urgent symptom management needs at the time of admission
- Communicating findings, recommendations, and care needs to the interdisciplinary team
On-Call & Urgent Clinical Support
- Respond to urgent and same-day hospice admissions during assigned on-call hours
- Respond to urgent visit needs, including symptom escalation or time-sensitive clinical concerns
- Provide clinical backup support to field staff as needed
- Support smooth transitions of care, including hospital discharges to hospice and changes in level of care (e.g., GIP)
Coordination & Communication
- Interface and coordinate closely with Intake, referral sources, care partners, and interdisciplinary team members
- Communicate effectively and compassionately with patients, families, caregivers, and facility staff
- Ensure timely, accurate, and complete documentation in the electronic medical record in accordance with hospice regulations and organizational standards
Qualifications
- Current, active Registered Nurse (RN) license in good standing (state-specific as required)
- Hospice or palliative care experience strongly preferred
- Acute care, hospital-based admissions, or on-call nursing experience preferred
- Strong assessment, organization, and clinical decision-making skills
- Ability to work independently and respond effectively in time-sensitive and emotionally complex situations
- Valid driver’s license and reliable transportation
Compensation
- $250 flat daily on-call rate for assigned on-call days
- Hourly RN rate paid for all hours worked (admissions, urgent visits, and clinical support) in addition to the flat on-call rate
Physical & Other Requirements
- Ability to travel to multiple care settings throughout the King County area
- Ability to perform patient assessments and provide direct nursing care as required
