Job Description
Our home health and hospice agencies are located throughout California, Hawaii, Oregon and Washington.
They serve the entire community and offer personalized in-home care, and many have been recipients of Home Care Elite awards.
Job Summary:
Coordinates incoming office referrals and verifies insurance coverage.
Coordinates incoming calls from patients, family, physicians, and disciplines.
Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals.
Coordinates small program(s) with limited budget/impact.
Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as a working supervisor for team or work group.
Day Shift 5 x 8s, M - F 8 am to 5 pm.
Must have LVN certification.
At least 1 year of experience.
This will be a clinical intake coordinator for home health processing and scheduling referrals. No Call.
A possible job title for this role could be LVN Clinical Intake Coordinator.
Candidates must have their LVN.
Education and Work Experience:
High School Education/GED or equivalent: Required
Associates/Technical Degree or equivalent combination of education/related experience: Preferred
Three years' relevant healthcare experience: Preferred
Pay: $32.00 - $34.00 per hour
Expected hours: 40.0 per week
