What are the responsibilities and job description for the Life Enrichment Coordinator position at Kirkhaven?
We are seeking a Full Time Life Enrichment Coordinator to plan, organize, and facilitate large and small group activities and socialization to enrich the lives of our elders.
This position is full time and requires weekend and holiday availability.
Primary Job Duties include:
- Plan, implement and evaluate large and small group activities and socialization to enrich the lives residents allowing them to communicate their own preferences and participate by their own decision in accordance with Kirkhaven’s person-centered philosophy. Activities should serve to provide socialization, enjoyment, engagement, entertainment, education and overall enrichment and quality of life.
- Contribute to monthly calendar of planned activities with Activities Manager. Distribute monthly calendar to staff. Post monthly schedule in lobby areas and resident rooms.
- Assess new residents on assigned units, develop and document a plan to meet their needs, interests and abilities through cognitive, social, emotional, fine motor and gross motor LE activities. Maintain care plan on assigned patients/residents.
- Complete personal profiles for each resident in Point Click Care. Ensure required documentation including assessment, progress notes, and attendance records is complete and timely and in compliance per established policies, codes and regulations. Monitor and record patient/resident progress/decline through quarterlies, progress notes and MDS. Attend and participate in care conferences for assigned patients/residents.
- Attend weekly care planning meetings, interdisciplinary meetings and (morning stand up as needed)
- Consult with clinical care team regarding patient/resident abilities, limitations, changes as related to LE participation. Communicate as necessary with Dining Services.
- Implement daily programming. Provide necessary materials. Encourage participation. Physically transport resident to and from activities. Encourage patients/residents.
- Plan and implement monthly van trips into the community. Select appropriate residents, make arrangements with nursing, dining, finance, drivers and volunteers as needed to be successful.
- Collect photos and video content for Regional Creative Director.
- Reports to the Activities Manager and Regional Creative Director
Qualifications include: BS in recreation/Leisure studies, music therapy, art education or related field or AAS or equivalent experience; Preferred minimum of 1 year experience in recreation and leisure program delivery in long term care.
Pay: $20.00 - $22.00 per hour
