This position supports KCHC’s mobile medical operations and community outreach programs in the Community Outreach Department. The role provides direct patient care and clinical support under the supervision of licensed providers while also addressing broader social determinants of health. Duties include clinical tasks (vital signs, draws blood, medication administration, and specimen collection), patient education, and outreach activities such as home visits, health screenings, and case management. The goal is to bridge healthcare and community resources to improve health outcomes, promote equity, and support the mission of KCHC.




Qualifications


Education:



  • High school diploma or equivalent required; Associate’s degree preferred.

  • Completion of an accredited Medical Assisting program preferred.


Certifications & Licenses:



  • Certified or Registered Medical Assistant required and in good standing.

  • CPR/BLS certification (or ability to obtain within 6 months).

  • Valid NC driver’s license with acceptable driving record and eligibility for company insurance.


Experience:



  • 1–3 years in a clinical or healthcare setting preferred.

  • Bilingual (English/Spanish) preferred

  • Experience in healthcare, nonprofit, or community-based organizations.

  • History of working with individuals and families from diverse cultural and socioeconomic backgrounds.


Knowledge, Skills, Abilities:



  • Strong written and verbal communication skills.

  • Ability to work collaboratively with internal departments and community partners.

  • Proficient in Microsoft Office Suite

  • Ability to safely and efficiently operate mobile unit systems and support clinical staff in patient care delivery.

Essential Duties and Responsibilities


     Clinical & Administrative Duties



  • Maintain strict confidentiality regarding patient records while utilizing a team-based approach to care.

  • Schedule/reschedule appointments, conduct patient calls, and assist with transportation as needed.

  • Accurately measure and record patients' vital signs; obtain histories and chief complaints and assist with drawing blood and other necessary screenings/tests.

  • Prepare patients for examinations and perform routine screening tests in the clinic or on the mobile health unit.

  • Accurately prepare and administer medications and injections under appropriate practitioner authorization and within scope of practice.

  • Collect and prepare specimens; sterilize and maintain instruments and exam rooms on mobile unit.

  • Support provider efficiency by performing delegated administrative tasks and EHR documentation.

  • Ensure compliance with OSHA, HIPAA, and KCHC policies.


     Outreach & Community Health Duties



  • Maintain a patient caseload and provide case management and navigation services.

  • Conduct outreach through home visits, health screenings, community events, and mobile health clinics.

  • Educate patients and community members about KCHC services, including sliding fee programs.

  • Assist with in developing mobile outreach schedules, ensuring there is a variety of patient payor mix and performance summaries

  • Research and connect patients to community-based resources based on individual care plans.

  • Accurately document outreach activities, charge entries, and schedules in NextGen or assigned systems.

  • Attend community meetings, internal trainings, and professional development opportunities.


 


 


Key Performance Indicators (KPI)



  • Number of community outreach events supported annually

  • Number of patients engaged through mobile medical clinics

  • Number of new patients successfully linked to a KCHC primary care provider

  • Number of referrals made to social services and community resources

  • Completion rate of follow-up contacts with patients after mobile clinic visits

  • Accuracy and timeliness of Outreach documentation in Nextgen




  


Work Environment & Schedule



  • Must be able and comfortable working in a variety of settings including, but not limited to clinical environments and office spaces.

  • Participation in community events may involve outdoor work and exposure to varying weather conditions, including high or low temperatures.

  • Must be able to work nights and weekends as departmental needs arise.

  • Must be able to operate all company vehicles safely during both daytime and nighttime hours.


 


 


Travel Requirements



  • Local travel may be required for community events, outreach activities, meetings, and organizational initiatives.

  • Must possess a valid driver’s license and maintain a driving record that meets organizational insurance requirements.

Physical Demands



  • Prolonged periods of sitting while working at a computer and viewing screens.

  • Frequent use of hands and fingers for typing, writing, and operating office and medical equipment.

  • Requires close visual acuity to perform activities such as viewing computer screens, preparing and reviewing documents.

  • Occasional standing and walking, particularly during meetings, events, or outreach activities.

  • Ability to move throughout office and event spaces as needed to perform job duties.

  • Ability to bend, reach, and set up materials for events or outreach activities as needed.

  • Occasional lifting, carrying, or moving materials such as marketing supplies, signage, or event equipment weighing up to 25–50 pounds.

  • Assisting patients with positioning, transfers, and mobility (e.g., helping on/off exam tables).

  • Fine motor skills required for clinical tasks such as administering injections, performing venipuncture, handling medical instruments and equipment.

  • Regular exposure to blood and bodily fluids, infectious diseases. Use of appropriate PPE required.

  • Must be able to operate all company vehicles safely during both daytime and nighttime hours.
