In this role, you will:
- Conduct a comprehensive assessment of individuals in their homes.
- Develop individualized care plans based on assessed needs.
- Provide in-service training to Direct Support Professionals (DSPs) on individual care plans and best practices.
- Conduct monthly check-ins with individuals to monitor their well-being and ensure quality of care.
- Maintain accurate and up-to-date client records.

Qualifications:
